fill out this Enrollment Form and submit it to us. Upon completion of the form, you will
be given on-line or mailing payment choices. You may pay the full $695 course
tuition and $100 materials fee.
(on-line) You may pay a deposit of $200 in advance to hold your place. (on-line)
You may pay the balance due on your account. (on-line) Or you may choose to mail a check
if you prefer. This form and payment must be received no later than the first of the month
preceding the workshop date. This will guarantee that your reservation will be held for
you.
